Edit Org Package Ref

Submission from the inline "Edit" dialog in a row of the Rate Setups (By FD) institutional billing list. The fields are largely identical to "Add Institution Package", the difference being that editing uses default_id to locate the record and no longer submits org_id (the institution cannot be changed).
